Mailing Options
When calling, you will be asked to provide the following information:
- Decedent’s full name
- Date of birth
- Social Security Number
- Place of death (U.S or Foreign)
- Policy or Contract number (if known)
- Date and Cause of death
- Name of person reporting the claim, and his/her relationship with deceased
- Daytime and evening telephone numbers
- Mailing Instructions for claim packets
